  • Varicella (Chickenpox):
    • All matriculated students are also required to submit vaccine records or blood test results for varicella (chickenpox).
    • Please note that a second dose of varicella vaccine is now required.
    • For individuals reporting prior natural illness, health care provider verification of illness is accepted if the student was born in the United States.
    • Only individuals born in the United States before 1980 will be exempted from the varicella requirements.
  • Meningitis:
    • Residential students must have a meningitis vaccination on file before taking occupancy of campus housing. A booster dose is required if the first dose was given more than 5 years before the student's entry date.

  • Health professions students (nursing, pharmacy, physician assistant and dietetic interns) have additional health requirements; please review the health profession specific physical and vaccine form and department materials for further details.
  • Health sciences students are not eligible for date-of-birth related immunization exemptions for MMR and varicella requirements.
