undergraduate students, graduate students, faculty and staff.

How to request delivery of a prescription:
Students must set up an account with Maple Pharmacy (by phone) to include:
- First and last name
- Date of birth
- Mobile number (of student)
- Home address
- Insurance information
- Name of medication
- Form of Payment
Completing Health Forms
Required health forms are completed and submitted through myHealth, the student health portal. The portal is accessible to current students and to incoming students approximately 24 – 48 hours after receiving USJ credentials.
You will upload your immunization record and all other supporting documentation to myHealth as part of your form completion. For most students, there are no paper forms to complete.
- STEP ONE: Access the portal by clicking the myHealth logo above, the myHealth Quick Link found on the left side of the landing page of MyUSJ, or by visiting usj.studenthealthportal.com.
- STEP TWO: Log in to myHealth with your USJ credentials and select theMy Forms tab to begin completing the required health forms.

Fall Semester [classes begin Aug., Sept. or Oct.]: July 15
Spring Semester [classes begin Jan., Feb. or March]: December 1
Summer Semester [classes begin May, June or July]: April 1
Students should adhere to any deadlines set by their academic program as they supersede and may differ from the deadlines listed above.

Special Circumstances
Health Insurance
Health insurance is recommended for all students and may be required for some students.
USJ does not currently offer a health insurance program for domestic students, so no waiver or proof of insurance is required for enrollment. Students will be notified if there are any changes in health insurance requirements.
Due to their specific needs, and within the context of rapidly increasing US health care costs, international students are automatically enrolled in a University-sponsored program. Updated rates will be available after July 1, 2025.
