USJ Student Health Services at The Beekley Center for Student Health and Wellness offers confidential primary care services to enrolled undergraduate and graduate students and is located on the first floor of the O’Connell Athletic Center.
Health Record Requirements
Accepted incoming students can download required health forms by visiting MyUSJ:

All matriculated USJ students are required to provide proof of adequate protection against measles, mumps, rubella, and varicella.
Students planning to live on campus are required to provide documentation of a Meningococcal ACWY (“meningitis A”) vaccination within the 5 years prior to their arrival on campus.
Health professions students have additional health documentation requirements to fulfill prior to beginning any clinical practicums.
Student athletes must provide additional documentation as required by the Department of Athletics prior to participation.
Health insurance is required for student athletes and students in the following programs: nursing, pharmacy, physician assistant, and dietetic internship.
Health insurance is recommended for all other students.
